  1. O acidente do 9501 não foi provocado por ingestão de FOD nos motores. Durante a execução de um treino de carga suspensa, o cabo que sustentava a carga enrolou-se no rotor de cauda, e provocou a perda de controlo do helicóptero por parte dos pilotos. O subsequente embate da aeronave com o solo causou 1 morto e 3 feridos, e não 5 feridos.
